Our agency encountered youth and adults who were in need of positive support systems. Their needs were the motivation to start this agency and provide services and resources for children, adults, and their families.

Peer Support Specialist

The Peer Support Specialist (PPS) operates within the framework of recovery and includes: coordination and oversight of initial and ongoing assessment activities; initial development of the PCP; monitoring and implementation of PCP; additional case coordination functions for linking, arranging for service and referrals; relationship building; therapeutic engagement; supervision of Associate Professionals, Paraprofessionals, and Peer Specialists working with the team; coordination and implementation of 24/7/365 first responder crisis responses; supportive counseling; individual and group counseling; recovery skill building; symptom management/monitoring; and empowering individuals to find and identify their own goals and to assist in developing the necessary lifelong knowledge and skills to function on a daily basis.

Medical Records Specialist

The Medical Records Specialist is responsible for ensuring the medical records are properly organized and meet the standards set forth by the Agency and its authorizing bodies. This position will serve as a role model to support staff, by consistently conducting him/herself in a professional and ethical manner in all situations, including but not limited to promptness of completing assignments, verbal/non-verbal communications, maintain professional boundaries, representing the Agency with a professional image, complying with Federal Anti-Kickback Laws, and not engaging in or coercing others to engage in any fraudulent behavior (see Federal Anti-Kickback Laws and Medicaid Fraud Defined).

CMA/Health Home Care Manager

This role involves providing direct services to individuals with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or Children with SED. Alternatively, it includes connecting these individuals to a wide range of essential community services, such as medical, psychiatric, social, educational, legal, housing, and financial support. Responsibilities encompass assessing clients’ situations through interviews and on-site visits, setting goals, facilitating referrals to community resources, interpreting behavior patterns, maintaining case records, and conducting follow-ups. Additionally, the role requires compliance with legal requirements, safeguarding client confidentiality, and empowering individuals to identify and pursue their own goals while building essential life skills for daily functioning.

CMA/Health Home Care Manager Extender

The role emphasizes maintaining confidentiality, following procedures, and empowering clients to identify their own goals and develop essential lifelong skills for daily functioning.

AP (Associate Professional)

Ensure that the Community Support Service operates within the framewmk of recovery and includes: coordination and oversight of initial and ongoing assessment activities with supervision; assist in initial development of the PCP; assist in monitoring and implementation of PCP; additional case coordination functions for linking, affanging for service and referrals; relationship building; therapeutic engagement; supervision of Paraprofessionals working with consumers; coordination and implementation of24/7/365 first responder crisis responses; supportive counseling with supervision; individual and group counseling with supervision; recovery skill building; symptom managefnent/ monitoring; and empowe1ing consumers to find and identify their own goals and to assist consumers in developing the necessary lifelong knowledge and skills to function on a daily basis.

LP (Licensed Professional) Community Support Team Leader

Ensure that the Community Suppmt Team operates within the framework of recovery and includes: coordination and oversight of initial and ongoing assessment activities; initial development of the PCP; monitoring and implementation of PCP; additional case coordination functions for linking, arranging for seivice and refen11ls; relationship building; therapeutic engagement; supervision of Associate Professionals, Paraprofessionals, and Peer Specialist working with the team; coordination and implementation of24/7/365 first responder crisis responses; supportive counseling; individual and group counseling; recovery skill building; symptom management/ monit01ing; and empowering consmners to find and identify their own goals and to assist consumers in developing the necessacy lifelong knowledge and skills to function on a daily basis.

Team Leader Intensive In-Home

Certified Drug and Alcohol Counselor

A Certified Drug and Alcohol Counselor (CDAC) engages in a comprehensive range of Twelve Core Functions, encompassing essential aspects of addiction counseling. These functions include screening, intake, orientation, assessment, the formulation of treatment plans, counseling sessions, case management, crisis intervention, client education, meticulous reporting and record-keeping, collaboration with fellow professionals regarding client treatment and services, and making referrals for the treatment of addictive disorders while also focusing on relapse prevention strategies.

Licensed Therapist

In this role, the counselor assesses clients through interviews, medical, psychological, and social evaluations, and on-site visits. They collaborate with clients to set goals and explore options, facilitating referrals to community resources and establishing rapport with other agencies. The counselor interprets behavior, maintains case records, and conducts follow-ups, while adhering to policies and legal requirements. They prioritize client confidentiality and offer supportive counseling, individual and group therapy, recovery skill development, symptom management, and empowerment for clients to identify and pursue their lifelong goals while functioning effectively in daily life.
